Session rename reverts

Rename the session from the activity-bar session list instead of the chat header.

Run /rename <same name> again after resuming to restore the name reported by ListAgents

AI title overwrites session name

Set CLAUDE_CODE_DISABLE_TERMINAL_TITLE=1 in the session's environment so no AI title is generated, or set the title with /rename <name>
