Missing didSave on first edit
“When Edit changes a file that the LSP server doesn't have open yet, Claude Code sends only textDocument/didOpen, carrying the already-edited text.”

First edit of a file sends didOpen without didSave. On Edit or Write of a file the language server hasn't opened, only didOpen is sent, while later edits send didChange plus didSave.
