apiKeyHelper not re-run on 401 in Vertex gateway mode. With CLAUDE_CODE_SKIP_VERTEX_AUTH=1, an expired cached helper token gets a 401 and every request keeps failing until CLAUDE_CODE_API_KEY_HELPER_TTL_MS elapses.
