Survey ignores opt-out settings

Session quality survey still appears despite opt-outs in desktop Code tab. The "How is Claude doing this session?" prompt keeps appearing with CLAUDE_CODE_DISABLE_FEEDBACK_SURVEY=1 set, and with feedbackSurveyRate: 0 in one report.
